PROPOSED LIST OF COMPETENCIES FOR REPORTING OF
MILESTONES TO ACGME FOR PEDIATRIC SUBSPECIALTIES
The ACGME and ABP, with input
from CoPS, have developed the following proposed list of Pediatric Competencies
for which Pediatric Subspecialty Programs will report Milestones to the ACGME
beginning December 2014. In order to reduce workload burden, the ACGME is
only requiring reporting on a subset of the ~50 Pediatric Competencies.
While the residency programs are reporting on 21, this same list is not
appropriate for the Pediatric Subspecialty Programs, since several competencies
not on the list were more applicable to fellows. This draft was developed based
on 1) identifying those competencies more pertinent to subspecialists (e.g.
acting in a consultative role), 2) including “hot topic” competencies
(e.g. transfer of care), and 3) allow the ACGME to track competencies as
the trainee advances through residency and fellowship (e.g. professional
conduct). The list includes:
1.
Provide transfer of
care that insures seamless transitions (PC3)
2.
Make informed
diagnostic and therapeutic decisions that result in optimal clinical judgment
(PC6)
3.
Develop and carry
out management plans (PC7)
4.
Provide appropriate
role modeling (PC12)
5.
Critically evaluate
and apply current medical information and scientific evidence for patient
care (MK2 combined with PBLI6)
6.
Identify strengths,
deficiencies, and limits in one’s knowledge and expertise
(PBLI1)
7.
Systematically
analyze practice using quality improvement methods, and implement changes with
the goal of practice improvement (PBLI4)
8.
Use information
technology to optimize learning and care delivery (PBLI7)
9.
Participate in the
education of patients, families, students, residents, and other health
professionals (PBLI9)
10. Communicate
effectively with physicians, other health professionals, and health related
agencies (ICS3)
11. Work effectively as
a member or leader of a health care team or other professional group
(ICS4)
12. Act in a
consultative role to other physicians and health professionals
(ICS5)
13. High standards of
ethical behavior which includes maintaining appropriate professional boundaries:
Professional Conduct (PROF2)
14. Work effectively in
various health care delivery settings and systems relevant to their clinical
specialty (SBP1)
15. Coordinate patient
care within the health care system relevant to their clinical specialty (SBP
2)
16. Incorporate
considerations of cost awareness and risk-benefit analysis in patient and/or
population-based care as appropriate (SBP3)
17. Work in
inter-professional teams to enhance patient safety and improve patient care
quality (SBP5)
18. Participate in
identifying system errors and implementing potential systems solutions
(PBLI6)
19. Demonstrate
trustworthiness that makes colleagues feel secure when one is responsible for
the care of patients (PPD5)
20. Provide leadership
skills that enhance team function, the learning environment, and/or the health
care delivery system/environment with the ultimate intent of improving care of
patients (PPD6)
21. Recognize that
ambiguity is part of clinical medicine and respond by utilizing
appropriate resources in dealing with uncertainty (PPD8)

We would greatly appreciate
your input
regarding the selection of these competencies, including any suggested additions
or deletions. Once feedback is received and incorporated, the list will be
reviewed by the Pediatric Residency Review Committee of the ACGME for approval
with the goal of having it finalized by mid-November 2013. Note that the
first reporting of Milestones to the ACGME by Pediatric Subspecialty Programs
will be in December 2014.
